Easy access to care

• $0 Virtual First: on select plans, members pay $0 for telehealth and virtual visits for primary care, behavioral health and urgent care, including consultation, diagnosis, follow-up care and prescriptions.
• Chronic condition support: members with chronic conditions can get self-monitoring tools, such as a blood pressure monitor or scale, at no additional cost, with clinician support and regular reminders.

Ways to save

• $0 maintenance medications with a simple formulary design.
• Quartz Gold Maintenance plan: personal durable medical equipment such as CPAP and supplies, hearing aids and nebulizers with no out-of-pocket cost. Other plans cover DME with fixed coinsurance. Prior authorization may apply.
• HSA option: certain 2026 Bronze and Catastrophic plans can be paired with a Health Savings Account, including all on-exchange and most off-exchange Bronze plans, even those with a $0 deductible or copays, under new federal tax guidance.

Choose a primary care provider

Every member should pick a primary care provider or primary care clinic. If you don’t choose one, Quartz assigns you one: members in Dane County go to the UW Health Welcome Center, and members elsewhere get a clinic near home. You can change it at any time, and family members can each pick their own. PPO plans don’t require a PCP.

No referral required

You don’t need a written referral to see an in-network specialist, though Quartz recommends checking with your PCP first. Some health systems require a primary care visit before you book with a specialist.

Virtual care

Members can get virtual care through UW Health Care Anywhere, Emplify Health by Gundersen MyChart or the LiveWell app from Advocate Aurora Health. On plans with Virtual First, in-network virtual primary, urgent and behavioral health visits are $0.

Preventive care

Most preventive services, including screenings, wellness visits and most immunizations, are covered at no extra cost. If a visit or test is to evaluate symptoms or diagnose a condition, it is diagnostic and cost sharing may apply.
